Book of remembrance order form

Please read before starting your application

To add an entry into the Book of Remembrance, you will need to go through the following steps:

Step 1 - choose date of entry (date 1-31, month Jan – Dec)

Step 2 - choose the type of inscription you would like

Step 3 - choose the wording for your inscription

Step 4 - choose an emblem to go with your inscription (optional). If you choose to have an emblem, you can select from our catalogue of images or choose your own. If choosing your own, you will need to describe an image for the artist to draw or upload an image of your choice. Examples of entries can be viewed here to help with all of the above steps.

You will need to make payment using a debit or credit card.

Step 1 - choose date of entry

We will only permit entries on 11th November, 25th December, 1st January and 14th February where it is the anniversary of date of death or birth date.

Please type the day followed by the month in the box, eg. for 9th July, type 09/07.

Step 2 - choose the type of inscription

You can view examples of entries to help decide what option you would like. Select the options below for information on prices.

Applications must be received at least 6 months before your required date of entry. Any applications received after this deadline may not appear until the following year.

Entries received beyond this time will appear in subsequent years’ displays’.

Step 3 - choose the wording for your inscription

Please read before completing

  • Line 1 has a maximum of 28 characters, Line 2 a maximum of 30 characters, and Line 3 onwards has a maximum of 35 characters
  • Entries must have of a minimum of 2 lines
  • Line 1 must consist of the name of the deceased only and must be surname followed by forename/s

Please enter the required inscription without worrying about word or line breaks. Our calligrapher will make any necessary adjustments for a seamless presentation
